Director of Emergency Services & Care Transitions


This is a Contract position in Sarnia, ON posted November 22, 2022.

The successful applicant will work in compliance with all relevant and applicable legislation and Hospital Policies and Procedures, will practice the Bluewater Health Standards of Performance Excellence and will fulfill their role within the Attendance Support Program parameters. The successful applicant will also possess effective communication and interpersonal skills, will have the ability to work independently and as part of a team, and will have strong attention to detail and accuracy.

• Bachelor degree in nursing or related health discipline required
• Current registration with a Health Discipline College required
• Master’s degree in nursing or related field required

Skill, Ability and Experience:

• At least five (5) years’ experience in acute hospital service required
• At least five (5) years’ experience in management/leadership position required
• Knowledge/Experience of Emergency Department workflow and processes in the past 5 years preferred
• Demonstrated ability in day-to-day problem solving (apply critical thinking, decision making, conflict/complaint resolution, fiscal budget and performance management, change management, accountability and risk management) required
• Ability to use related information systems tools (Meditech, Staff Scheduling, MS Word, Outlook, Excel) required
• Ability to lead and implement change effectively with a key focus on patient safety and quality required
• Demonstrated ability to work collaboratively within a multidisciplinary and inter professional care team required
• Demonstrated ability to promote staff development, quality improvement programs and patient and family-centred care required

Principal Duties:

• Promotes Bluewater Health’s mission, vision and values
• Responsible for BWH Strategic plan initiatives
• Portfolio includes Emergency Department, Sexual Assault & Domestic Violence program, Patient flow and Care Transitions, Alternative Level of Care (ALC) and Clinical Resource Team
• Leads process improvement initiatives taking into account best practices, innovative opportunities to align with strategic direction including with Ontario Health Team (OHT) strategies
• Promotes and facilitates the use of systems that ensure patient needs are being met, and that the provision of patient care is consistent with BWH strategic priorities and with the goals, objectives and standards of the programs
• Facilitates and champions an interdisciplinary approach to problem solving and decision-making by the patient care team around program and patient care issues
• Monitors and analyses the results of quality measurements and recommends solutions/changes to improve patient care/outcomes
• Supports and directs day-to-day utilization of human resources and beds to achieve the program’s goals and function in collaboration with leadership teams
• Monitors attendance and performance of team members in achieving the disciplines’ standards and performance management
• Responsible to achieve the program financial targets by monitoring program expenses and implementing corrective action
• Responsible for benchmark targets related to patient satisfaction, employee satisfaction, leadership development initiatives, quality improvement initiatives
• Utilizes data to measure quality of care, patients and staff satisfaction, clinical and financial performance outcomes
• Works collaboratively with Professional Practice, Clinical Directors, Corporate Directors, Performance Management
• Monitors compliance with all relevant legislation, corporate policy, and collective agreements
• Provides support, guidance and mentorship in terms of decision-making around hospital processes relating to discharge and transition of patients both within the organization and extending into the community
• Supports and directs all aspects of discharge planning, ALC performance, ALC reporting in collaboration with manager Care Transitions
• Collaborates with Home and Community Care partners, Long term Care partners, Red cross, and other community partners
• Supports/Leads NOW Initiatives to improve access to care, Estimated Discharge Date strategies, and other projects
• Ensures appropriate adherence to the hospital discharge policies in order to maintain throughput, resulting in maintaining access to emergency services, meet the hospital’s commitments, and match patient needs with the appropriate environment and clinical support
• Participates in team building and promotion of good morale within the teams
• Quality Improvement Initiatives related to Discharge/ Transition planning and practices
• Works collaboratively with the Patient Flow Coordinators, Clinical Managers, Physicians, and Home and Community Care, to ensure seamless transitions, discharges, access and flow throughout the organization and into the community
• Responsible for ensuring all ALC strategies, utilization, and reporting are met
• Engages in situational management and conflict resolution strategies
• Ensures that all situations handled are documented and forwarded to the appropriate Director/Manager or Medical Director
• Other related duties as assigned
