The successful applicant will work in compliance with all relevant and applicable legislation and Hospital Policies and Procedures, will practice the Bluewater Health Standards of Performance Excellence and will fulfill their role within the Attendance Support Program parameters. The successful applicant will also possess effective communication and interpersonal skills, will have the ability to work independently and as part of a team, and will have strong attention to detail and accuracy.

-Masters Degree in Social Work from an accredited school of Social Work required
-Registered MSW and member of Ontario College of Certified Social Workers and Social Service Workers required
-Basic proficiency in the use of computers and electronic documentation systems required
-Minimum two years recent experience in social work within a hospital setting preferred
-Experience in providing psychosocial assessments, counseling, patient education and group facilitation preferred
-Provide social work services to patients/families including psychosocial assessments, adjustment/crisis, bereavement, supportive, palliative and resource counselling, conflict resolution, patient education, advocacy and referral services
-Discharge planning with internal and external partners to facilitate smooth transitions in care
-Statistical data collection, reporting and analysis
-Crisis/Trauma response as a member of the Critical Incident Stress management team
-Participates in unit/department and Hospital Quality Improvement initiatives
-Completes continuing education requirements as necessary
-Other related duties as assigned
