The successful applicant will work in compliance with all relevant and applicable legislation and Hospital Policies and Procedures, will practice the Bluewater Health Standards of Performance Excellence and will fulfill their role within the Attendance Support Program parameters. The successful applicant will also possess effective communication and interpersonal skills, will have the ability to work independently and as part of a team, and will have strong attention to detail and accuracy.

-Current member in good standing with the College of Dietitians of Ontario required
-Successful completion of dietetic internship program accredited by Dietitians of Canada required
-Current registration as a Certified Diabetes Educator preferred
-Current membership in Dietitians of Canada preferred
-Motivational Interviewing Training preferred
-Certified Insulin Pump Training preferred
-Demonstrated skill in nutrition care in diabetes education (Type 1, 2, GDM, Pre-Diabetes), eating disorders, renal, and mental health with paediatric, adult, and geriatric patient populations) preferred
-Experience with glucometers, insulin adjustments, and insulin pump education preferred
-Capacity to work with a multidisciplinary patient care team required
-Demonstrated competence with Microsoft Office and Microsoft Outlook required
-Demonstrated experience working with electronic medical record required
-Triage and arrange appropriate referrals in collaboration with other dietitians.
-Patient Nutrition Assessments
-Evaluate patient lab results pertinent to nutrition intervention
-Calculate nutrition requirements
-To identify patients’ need for follow up and perform patient follow ups in person, by e-mail or by telephone
-Identify education needs and assess readiness to change of patients and family members
-Develop appropriate teaching plans, provide education, and evaluate learning
-Participate in patient care interdisciplinary team meetings
-Routine communication with administration, food services, allied health, and physicians.
-Employ information technology appropriately for patient care (i.e. statistical tracking)
-Document patients’ nutrition care in patients’ records
-Stay current and aware of opportunities to implement new, evidenced-based methods of patient assessment and treatment
-Willingness to attend professional development opportunities (including weekends and evenings)
