This is accomplished across four geographic management zones which are responsible for the operation of acute care health centres and the provision of a variety of inpatient, outpatient services including academic, tertiary, quaternary care and community based programs and services including continuing care, primary health care, public health, and mental health and addictions.

Responsibilities Reporting to the Health Services Manager, the Care Team Assistant (CTA) works under the direction and supervision of the Licensed Practical Nurse (LPN) and/or Registered Nurse (RN), to perform direct and indirect resident care tasks and support functions.

Those assigned tasks and functions support the role of the professional staff and the efficient operation of the unit, and are delivered in a safe manner.

Assigned roles and responsibilities will be within the care delivery model and based on the needs of the patient population, the health care system, and the RNs and LPNs with whom the CTA works.

The CTA is also responsible for participating in cross-training for the CRC and Medical Unit in order to fufill responsibilities within these areas as required.

The CTA ensures that the principles and practices that guide our work such as the mission, vision, values, expected behaviours, the leadership philosophy, organizational health, population health, ethics, safety, quality, partnerships and interprofessional collaboration are integrated within the services they provide and through the messages they deliver.

Qualifications Graduate of Continuing Care Assistant Program Certified or eligible to be certified for Continuing Care Certification with Dept of Health and Wellness If not currently certified, must obtain within one year of hire Current BLS-C (HCP) certification Certification and/or training in areas applicable to the position such as Rehabilitation, and Geriatric, Dementia and Alzheimer Care Equipment specific education such as patient lifts (standing/floor/ceiling), bathing units (whirlpool tubs, showers) and other equipment as required Effective communication skills
– written and verbal Ability to work well in a team environment with demonstrated conflict resolution skills Knowledge and understanding of professional relationships Demonstrated organizational skills with the ability to prioritize tasks Knowledge of infection control practices, including use of personal protective equipment (PPE) and WHMIS Knowledge and understanding of the provision of patient/resident care within a hospital setting Excellent oral and written communication skills, with the ability to interact professionally with patients/residents, families, other health care providers and staff with a pleasant and cooperative attitude, demonstrating sensitivity, empathy and warmth Commitment to promoting a culture of respect that supports safety, ethical practices, cultural awareness and organizational health Ability to work in collaboration with multi-disciplinary teams within their assigned areas Capable to adapt and understand patients/residents’ needs, assisting them in meeting their fullest physical and emotional potential Demonstrates sound decision-making skills Basic computer skills
